*If you are unable to provide your insurance information at the time of your visit, you can be seen by making payment at the time of service. If you provide insurance information at a later date, we will be happy to refund any money due after the insurance claim is reconciled.

We accept most public and private insurance plans. We also offer special pricing for self-paying patients for office visits on day of service. Please call our office to confirm.

All new patients: please bring your drivers license or picture id and insurance card* if applicable to be scanned during check in.
Payment for insurance co-pay or if you intend to self-pay payment is due at the time of treatment.
Please arrive 10 minutes early to your appointment to fill out appropriate paperwork.
